How to Get Certified — The Two-Step Process

Qualifying for a Florida MMJ card requires two things: a physician evaluation confirming your qualifying condition, and OMMU state registration. Both happen quickly — most Miracle Leaf patients have dispensary access the same day as their evaluation.

1
Physician evaluation at Miracle Leaf
Dr. Samuel Sadow reviews your medical records, confirms your qualifying condition, discusses delivery methods, and enters your certification into the OMMU registry the same day. Evaluation takes under 30 minutes. New patient fee: $179.
2
OMMU state registration — $75/year
After your evaluation, log in to the OMMU patient portal at mmuregistry.flhealth.gov, upload your photo, and pay the $75 annual state registration fee. Your physical card arrives by mail in approximately 10 business days. You can visit dispensaries immediately using your patient ID number.
What to bring to your evaluation
Medical records documenting your qualifying condition diagnosis, a valid Florida driver's license or state-issued ID, a list of your current medications, and your payment for the evaluation fee. Not sure what records you need? Call us at (561) 888-6111 — we can advise before your appointment at no charge.

Frequently Asked Questions

What conditions qualify for a Florida medical marijuana card?

Florida Statute 381.986 lists: cancer, epilepsy, glaucoma, HIV/AIDS, Crohn's disease, Parkinson's disease, multiple sclerosis, PTSD, ALS, terminal conditions, and chronic nonmalignant pain. Physicians may also certify conditions of the same kind or class — including anxiety, depression, insomnia, and fibromyalgia when they occur alongside or as a result of a listed condition.

Does my condition qualify for a Florida MMJ card?

If your condition is listed in Florida Statute 381.986 — or is of the same kind or class as a listed condition — you may qualify. The only way to confirm eligibility is through an evaluation with a Florida-licensed physician. Call Miracle Leaf at (561) 888-6111 before your appointment — Dr. Sadow can discuss your situation at no charge.

Can I qualify if my condition is not on the list?

Yes, in some cases. Florida Statute 381.986 includes a provision for conditions "of the same kind or class" as the enumerated conditions. This allows physicians to certify patients with conditions that share clinical characteristics with listed conditions — evaluated on a case-by-case basis at your appointment.

How do I get certified for a Florida MMJ card?

Step 1: Physician evaluation at Miracle Leaf — Dr. Samuel Sadow reviews your records, confirms your qualifying condition, and enters your certification into the OMMU registry the same day ($179 new patient). Step 2: OMMU state registration at mmuregistry.flhealth.gov — $75 annual fee. Most patients have dispensary access the same day as their evaluation. See our full guide: How to Get a Florida MMJ Card.

Do anxiety and depression qualify for a Florida MMJ card?

Not independently. Anxiety and depression are not listed as standalone qualifying conditions under Florida Statute 381.986.
